Q: is this one cotton sleeve and 2 clay packs?

I think we ordered a two pack once and I believe it came with sleeves for both, but I’m not positive. Seems like the listing description should specify that. The sleeve doesn’t feel like it is made of cotton as you say in your question. My wife prefers to use it wrapped in towel. The main thing we like is that it isn’t hard like other ice packs.

2 clay packs and 2 cloth sleeves packaged separately.

Q: How well do they stay wrapped in place

Not well if you are moving around. It would depend on where you wrapped it, and if you use something other than the two skinny Velcro straps.

Q: The choices name 2 sizes.... but does that refer to the quantity of clay packs or the size? each looks like you get two of them but not sure...

They are by size. The extra large is 9.5X16 and the smaller one is 6X12. I only buy the extra large.
